2020 Hair Growth Therapies: A Look Ahead

In this article we will take a look at the most promising developments slated to occur in the hair restoration industry in 2020. 

Clinical Treatments And Drugs For Hair Loss In 2020

TissUse & J Hewitt – TissUse’s hair regeneration therapy is known as the Smart Hair Transplant or SHT which has been in development since 2011. In a familiar format, the SHT method seeks to take about 30 hair follicles from the back of one’s head and turn them into thousands of hair producing cells via culturing. In early 2019, the Japan-based company, J Hewitt, obtained a license to commercialize the SHT procedure. In September 2019, the CEO of J Hewitt announced his plans to conduct a 5 patient SHT trial by the end of the year. Due to complications in preparing the trial with a contracted cell processing company, the start date is now scheduled for late January 2020. J Hewitt’s CEO has made it clear he is committed to taking SHT to trial and proving whether it works in a timely manner. This initiative is possibly the most exciting development in the hair growth industry currently.

Follica – Follica’s FOL-004 treatment is scheduled to enter a pivotal FDA study in 2020. If successful, this final study would allow FOL-004 to become approved in the US and likely in Europe shortly thereafter. The first result photos from Follica’s wounding + topical combination surfaced on the internet in late 2019 and left audiences intrigued. An early estimation for FOL-004 to reach market approval is Q1 2021.

Samumed – The lengthy phase 3 trial of Samumed’s SM04554 is scheduled to complete in June 2020. The company will receive data from over 600 patients who applied its topical Wnt activator SM04554 on a daily basis for roughly a year’s time. Pending results, this trial could lead to the commercialization of SM04554 in Turkey and potentially the US. If approved, the prescription treatment would not reach patients until 2021 at the earliest.

Follicum – In their last major update, Follicum announced it is aiming to begin a phase 2 trial of its newly formulated topical FOL-005 peptide in January 2020. The trial will last around 3 months, so results from this important phase 2 should be available by summer 2020. This will be the first time the peptide will be trialed topically, so results from this trial are particularly important. Follicum’s FOL-005  will be looking to match or better results from its topical peers Breezula and SM04554.

Dr. Tsuji & Organ Technologies – It is anticipated that Dr. Tsuji’s fabled hair cloning therapy will enter its first clinical trial in 2020. A commercial release for this treatment in 2020 is extremely unlikely even with Japan’s expedited regulatory process. However, if Organ Technologies produces a successful initial trial in 2020, the ‘Tsuji hair cloning treatment’ could theoretically become available to patients in Japan within the next few years.

Stemson Therapeutics – The California-based hair regeneration company is within the scope of starting a human trial in late 2020. After launching their website and announcing an investment by Allergan in June 2019, CEO Geoff Hamilton told the media the company expected to begin trials in “about a year and a half.”

HairClone – HairClone continues to strive towards offering its expanded dermal papilla cell injections in the UK. Funding is required to create an MHRA licensed facility in which HairClone could prepare its cell therapy. Depending on the company’s ability to raise funds and create a licensed lab space, the DP cell therapy could potentially become available to patients in the UK by the end of 2020.

Cassiopea – The company developing the topical anti-androgen Breezula met with the FDA on November 13, 2019 to discuss the launch of a phase 3 clinical trial. The trial is expected to begin in Q1 2020 and is slated to last 6 months. As of now, results from this trial are estimated to read out in Q4 2020 and could lead to market approval in 2021.

Shiseido & Replicel –  Replicel has recently stated that it is engaging Shiseido about releasing results from the RCH-01 trial which took place in Japan. As the results were due in 2019, it is virtually impossible to think that the holdout can last much longer. One way or another we will get clarity or closure regarding Shiseido’s trial in 2020.

Cosmetic & Over The Counter Treatments In 2020

Giuliani Pharma – In September 2019, Giulani Pharma gave an exclusive update to Follicle Thought regarding the development of a WAY-316606 product for hair growth. We learned that the company decided to go for the cosmetic regulatory route and is researching several analog molecule equivalents to WAY-316606 to create a product with. According to a lead scientist at Giuliani, a small pilot human trial has been initiated with one of the candidate molecules and results are expected around the beginning of the new year. Regardless of how the first trial turns out, Giuliani has multiple molecules in their repertoire and is committed to seeing the research through. Look for an update on this development in Q1 2020.

Applied Biology – AB-103 is listed as a “minoxidil adjuvant therapy” on Applied Biology’s pipeline page. In late 2019, Dr. Andy Goren of Applied Biology told Follicle Thought his company is planning to release this product in the form of a shampoo. This shampoo is designed to help those who do not respond to minoxidil to get a response and is expected to enhance results in those who are already minoxidil responders. An estimated release of the product is Q3 2020.

Wild Card – There are always multiple companies quietly working behind the scenes on hair loss treatments. This is true at both the clinical and cosmetic level. However, the appeal of an effective cosmetic product is always higher due to the much shorter waiting period for such a product to reach the consumer market. When it comes to Wnt activating molecules and growth factor serums, there is potential to create an efficacious OTC product. Hopefully, in 2020 one or several companies who have been developing topicals over the last several years will finally unveil an exciting new product that will benefit many people.

      I would want to confirm with the company first, but those seem like reasonable dates. I would guess around September to know at least if the treatment works and results could improve past that timeline.

      Jeff, Aclaris did pretty well in their trials but they are looking for a larger pharma co to continue the development for Jak inhibitors in AGA and AA. At this point we’re awaiting news of a collaboration or imminent deal.

      This makes sense Russ, and another theory some ppl have said is that Shiseido is holding out on Replicel because they have a grudge about Replicel not fulfilling their end of the bargain. But if this was true, I don’t think they’d hold out forever. I give it 4 more months or so.

      Joe, I believe that theoretically a procedure like SHT should help if it does work. Adding the neopapillae to your scalp should improve your donor hair as well as your hair on top. Thinning hairs lack DP cells and adding the multiplied DP cells should thicken the hairs. It may be the case that someone with a thinning donor needs to get the injections done repeatedly to maintain the results, but I don’t see why it wouldn’t help.

    I reside in Saudi Arabia; I contacted the company in Saudi Arabia and assured me that the treatment of NGF37 is mainly the treatment of skin aging and is currently used in clinics in Jeddah by injection for hair growth and that the NGF-574H tonic with the same active substance from umbilical cord cells but with a greater percentage; also He assured me that the efficacy of the product is three times more than that of minoxidil

      Ahmed, I don’t believe the product was ever shown to be 3 times more than minoxidil. It would be great if the product was equal to minoxidil. The price of $150 for one bottle of tonic is certainly not inexpensive.

      John, the last we heard from Histogen they were seeking funding and mentioned during their investor presentation that they had funding which would carry the company into H1 2020. Hopefully they have secured additional funding by now or will soon. It would be optimal if Allergan would work something out with them and carry the HSC product through trials.

    You forgot to mention Histogen. Phase 3 in Mexico will end in june 2020, like Samumed. The treatment will be available in 2021 in Mexico only.

        It’s true, like Mattt said the phase 3 trial was never fully completed by Histogen’s partner in Mexico and eventually terminated. This was announced officially in a Histogen review article from earlier this year.

    Shenzhen, I heard briefly from the company but there is no current update about the baldness cure or gray hair treatment but it appears the company has posted on their Twitter account about creating a Gmp compliant culturing media for dermal papilla cells. This will be used for their future treatments and is available for sale to other companies in 2020 according to tweet.

    Should I hold off on a hair transplant? Was going to get one in 2020, but would waiting 5 years be better? Right now they have the DHI and Sapphire blade FUE. Are we on the cusp of changing how transplants are done?

      Well, there are several techniques which are looking to change the ability of hair transplants (to create unlimited donor or close to unlimited donor) but none of them should be available for consumers in 2020. We hope for the best, but we don’t know when they will actually be available. Waiting to see how one of the first clinical trials goes for the next gen treatments may be an idea for you. Make sure you weigh all of your options and give it a lengthy consideration.

I was going to get it in Feb, but maybe I should wait until 2021 and see what’s developed. Part of me wants to get it now, as I have the funds and time, but another side says wait it out.

      Hi, I don’t plan on emailing J Hewitt until late January if I don’t hear from him by them. I know he’s actively working on the trial with a third party company like he said he is and I’ll let him get it all sorted out.

      Next time I speak to him I will certainly ask how long the trial will last because this is an important question.

            What I find exciting about this article is not what it talks about but rather that hair loss cures are becoming a more mainstream topic – as this article was shown, and is still being shown in Google’s homepage as of me writing this comment.             It really doesn’t matter for me if hair loss cure is a mainstream topic or not.
            RCH-01 was tested by SHISEIDO and the results of Phase II will be posted soon or not.
            For me it only matters if this data is satisfied and SHISEIDO brings RCH-01 to market in 2020. the article you posted shows that dermal sheath cup cells are playing a significant role in hair growth so it is good

          There’s a difference between sheath cells and the sheath muscle. There is no evidence that injecting sheath cells will have any effect on the sheath muscle.

          Also, RCH-01 is cultured sheath cells, without DP cells, AND WITHOUT HAIR INDUCTIVITY RETENTION during culture. I do not see any chance of this working and I think that’s why Shiseido has been silent about their results.

            To be fair no one has proved yet that a dp cell is better than a ds cell. One or both could be good for growing hair if they are cultured properly. Your theory about shiseido could still be correct obv…

          Greg, thanks for sharing. I saw the article on Google as well and thought I had sent a reply to this earlier. This research is interesting and presents another potential target for helping hair grow.

          In this case, Rendl and his lab members think that applying drugs which prevent muscle contraction could keep hair growing and prolong the anagen phase. It’s a sound approach and will not be hard to test and prove. It sounds like something that Ralf Paus would be studying at his lab. Be on the lookout for repurposed drugs to be tested for this indication. Similar to WAY-316606 and its ability to inhibit SFRP1.

            Thanks for the input admin, but I really only follow cell therapies – not that I wouldn’t use a drug that gives you back all your hair and more. But IMO drugs will just always be “drugs” if you catch my drift, unless you’re talking about something like Exicure but I’m more looking forward to therapies such as TissUse and tsuji and that whole bunch. Realistically, which half of the industry do you think has a better chance in the future, or now?

              Greg, I agree that cloned follicles have a much greater capacity to reverse baldness and thus are more interesting to many people. But, I don’t count out drugs because preventing hair loss is a superior method than re-implanting cloned follicles to a lot of people. In the near term future I think additional drugs will help like SM and Breezula; TissUse has a shot to join the party, and in the more distant future both cloned follicles and drugs will do well. No one can really estimate which will come first between 1) a working cloned follicle therapy or 2) an extremely powerful drug which can either totally prevent hair loss or regrow a meaningful amount of hair.

      Alonso, Follica is a device which creates skin disruption at an in-office procedure. It supposedly only takes about 5 minutes at the office. Then, there is subsequent application of a topical (likely minoxidil, could be a mix as well). It’s somewhat unclear how much skin disruption follows in the at-home portion of the FOL-004 treatment.

      Too early to guess about Samumed photos in Sept, possible but I think the wait will be longer to be honest.

      PGD2 blocker angle is still viable, people had good results with topical setipiprant etc, but Allergan chose to trial a PGD2 blocker orally. There’s also the potential for better and more efficient PGD2 blockers to be developed as well. It’s similar to the differences between finasteride, dutasteride, and clascoterone (Breezula).

      In this case, the patent is just being updated with some new claim or adjustment which could be meaningful or not that important, it just adds IP protection to Cotsarelis. I think if setipiprant was trialed topically it would have been a success for Allergan.

    Sadly, I have to agree with Peter. I am starting to feel worn down with the breaking news and how some stories mention they have the cure, yet these “discoveries” seem to lay dormant for decades and don’t make it to market,

    I still check, but have learned to view it from a more skeptical perspective so I don’t need to constantly be let down.

      I would not suggest exposure to a Wnt inhibitor (oral or topical) without large, double blind toxicity and efficacy trials. Wnt is a massive cellular target involved in numerous biological pathways in many organs. The same goes for immunosuppressive targets and drugs like Cyclosporine A. Yes they can grow hair like mad, I have seen it, but they come with the risk of serious infection? Full head of hair with tuberculosis? No thanks.

      Topicals do go systemic and anyone that tells your otherwise is either clueless or not being forthright. Just read some of the hair loss board forums from men taking Brotzu and experiencing massive shedding without regrowth and in some cases anecdotes of feminization (i.e. budding breasts, testicular pain).

      What I look for is development of a peptide or small molecule targeting hair growth specific pathways and cell types, especially dermal cup progenitor cells. Still, the challenge with this approach is that we don’t fully understand the genetics of garden variety androgenic alopecia, which has proven to be a formidable challenge in finding a drug or drug cocktail that safely and robustly stimulate hair growth and overcome someone’s genetic endowment for hair loss. We have seen all too many amazing “results” photos with no real movement from pre-clinical testing.

      I think the next 2 years will be telling on the potential for development of oral and topical small molecules for treating hair loss. However, I fear that if these new targets/drugs fail to produce meaningful results in large trials, I am afraid investors and funding agencies will go dormant for some years before coming back to this in a robust way. Research also moves in business cycles and is very trend-driven.

      Cloning technology and organoid generation i.e. RIKEN, Tsuji, remains a big outlier here.
      If they can work out how to scale up this technology and demonstrate that cloned hairs behave as natural hairs after transplant in humans, this would be a game changer because it would then open up new ideas and drive that technology forward.

      My only issue with this approach is that it will still require surgery and the skill of the surgeon’s hands (and ego). I find the whole world of hair transplant surgery somewhat sleazy and fraught with risk and outdated techniques. There are plenty of patient self-reports of terrible unnatural looking transplants.

      Ideally, I would like to see a 21st century technology for hair loss remove the need for surgical intervention.

      In the mean time men should do whatever they can to hold on to the hair they have while we wait for the science to be ironed out. I have had a lot of success with topical finasteride compounded into minoxidil.

          It has been known for almost 20 years and well documented that within the dermal cup sheath of the dermal papillae exists a population of multi-potent stem cells which participate in maintenance and in some cases regeneration of the hair bulb.

          As with all regenerative medicine technologies, the challenge is to harness the potential of these cells ex vivo by scaling up the cell numbers while keeping them in a multipotent state. Generating large enough numbers (i.e. millions) of pure multipotent cells ex vivo is in itself a huge technical challenge. I worked on generating insulin producing beta-cells from stem cells for years, and yields of functional beta cells after ectopic implantation into the portal vein of the liver by me and others has remained unimpressive.

          In a pure stem cell population outside of their biological niche environment ex vivo, many cells will begin to spontaneously differentiate into any number of cell types in their multipotent program.

          I suspect that the unimpressive yields we have seen with RCH-01, and undoubtedly why Sheishedo is holding back their data, is because they have yet to figure out how to get a critical mass of DCS cells with undifferentiated stem cell populations ex vivo, AND equally important, figure out how to harness the regenerative program of these cells after reimplantation. The question always exists in the lab. “Ok we put the cell back in, but will they actually grow hair and how many”

          Regenerative medicine from the perspective of using the pluripotent potential of stem cells has not produced the translational therapies that were predicted 20 years ago for the reasons I point out as well as others on a tissue and disease-specific basis.

          I always say to my students, yes that was a long answer to a simple question, but science is always more complicated than yes or no. Right now I am not optimistic that RCH-01 injections will go anywhere anytime soon. I suspect Sheishedo will shelf this RCH project, at least in the public’s view, and go back to the drawing board on this technology if at all give the many other angles one can try to stimulate hair regrowth.

          Again, as a balding scientist, my advice is to do whatever you can to hold on to the hair you have (within reason) and watch as the next 2-3 years will be very telling as many of these small molecule trial results will have to put up or disappear into the dustbin of hair loss trial history.

          The alternative is just to accept your hairless and be free of this selfimposed obsession with having a full head of hair. Maybe we just need to ask ourselves on a deeper level why the obsession. I envy men who walk with a bald head and confidence. That is the best cure we all can hope for.

    Alonso, you are wrong . samumed has just completed the number of required participants which i assume is 625. And now all of them are using the topical solution, and a year mark is nothing but a relevant period from which they will conclude how great is sm04554 compared to existing topical aka minoxidil. by the end of next year you’ll know whether or not this thing works. One year is nothing to your hair follicles they will still be alive. just get on existing meds and you’d be alright.

      CRISPR is good at editing sequences of a single gene, with promising results for single-gene diseases such as sickle cell. However polygenetic conditions such as male pattern balding, where there are multiple genes at play, CRISPR is a long way off.

      Hi Hairguy, I have not heard much news about Kerastem’s phase 3, although the treatment is available in Europe which is a popular place for medical tourism and there doesn’t seem to be much of a buzz about it. Adipose-derived stem cell injections have been taking place in the US for some time now as well and there doesn’t seem to be a big response to those treatments.

Kerastem mention 17% increase hair growth but Follica 40% but both claim 30 cm2 hair growth. ? Confused on the math here lol. I hope Follica doesn’t end up like kerastem with little regrowth and all hype.

    @Admin happy new Year! Theoretical we should know much earlier if SHT works I think.
    If trials start in End of January then the participants give their hair biopsy and the cells will be multiplyied within 4 – 5 days. After that the surgeon will implant the SHT. So The neopapille will be injected in mid February 2020. I assume early results of this injection could be seen already in March 2020
    What do you think?

    SM04554 is a wnt pathway activator , and yet you claim that a wnt pathway activator is dangerous for our health. But , don’t you see the dosage of the wnt activator is very minimal and that it is still given as a topical treatment. Do you think that this type of danger is still to be possible even though the dosage is small ? Thank you for enlightening us 🙂

    I think this year is low-key a pivotal year for a lot of early stage MPB sufferers. If RCH-01 ever gets released and can prevent one from losing more hair on their head (even if it doesn’t regrow much new hair), people (like me) would get a new hairline transplanted and probably be happy with that. It’s discouraging to me that something exists right now that would literally change a lot of people’s lives but isn’t being released because of some ridiculous dispute. I want to have faith in RCH-01 but the lack of transparency is concerning to me, not trying to be negative. But, if at some point this year we can at least get some details and results from their trials that prove the products efficacy, and a statement of where the company plans to go from there, it would be encouraging. Basically, if RCH-01 does it’s jobs and stops hair loss, OR Tissuse’s SHT produces results that exceed the expectations of RCH-01, there will be a lot of excited people, and this should be a type of “cure” for a good portion of people with hair loss. Any estimated time for Replicel/Shiseido to release any info?

      We must be sure in case RCH-01 is released and know that it works also to rejuvenate miniaturized hair. If not, you may lose some of your miniaturized hair after a graft. I hope that Tissuse can do this and generate a significant percentage and once secured the miniaturized hair will return to its natural state, after this if a FUE graft would be an option.

    In my opinion, the most sensible route in treating hair loss would be trying to activate existing follicles again. However, many treatments under development seem to focus in moving new follicles to areas that have gone bald. If someone would figure out how to kick start miniaturized and dormant follicles again it would make surgery unnecessary and guarantee natural looking hairlines.
